Advocacy & Care Coordination
TMF facilitates a coordinated services team planning process in helping families
and individuals to advocate for themselves and access services in the community,
while preventing duplication of services from happening.
Support is given in learning to manage the system of community-based services
and programs, including liaison, referral and linkage with our partners
and other community resources. Families and individuals learn how to access
services and advocate on their own behalf.
