Conducting a Needs Assessment
The program conducts a needs
assessment and has in place assessment tools and processes for gathering
information from the individual and families from both programs that is useful
for helping to set goals and for measuring progress in becoming
self-sufficient.
Once the needs assessment is completed, a
Transition
Plan is prepared that includes individual and family goals, services and activities,
to guide service delivery during participation in the program, and during the
participant's transition to affordable housing in the community. The
participant's progress in attaining the goals and the services stated on the Transition
Plan is reviewed and evaluated on a weekly and
monthly basis.
The plan is revised as needed. The program also utilizes pre and
post
testing as a means for measuring outcomes gained while in the program.
An After-care Plan is developed prior to their transition into an
affordable home and their progress is tracked on a monthly basis.
